The Homeless Reduction Act 2017 puts more responsibility on Local Authorities to work with individuals to support them when faced with being homeless. There is a duty placed on public services to notify a Local Authority if they come into contact with someone who may be homeless.
You should be considered homeless if you have no home in the UK or anywhere else in the world for you to occupy. You don't have to be sleeping on the streets to be considered homeless and the Local Authority should consider you to be threatened with homeless if you are likely to become homeless within the next 28 days e.g. from eviction by landlord.
Establish as early on as possible if the patient has nowhere to go on discharge. It is not acceptable for the Ward to contact the Homeless Outreach Worker on day of discharge.
