
The Community Heart Failure Service provides specialist nurse support, and operates across Liverpool. This is a specialist service for patients with a diagnosis of heart failure.
The Community Heart Failure service provides specialist nurse support to adults diagnosed with Heart Failure confirmed by Echocardiogram.
The aim of the service is to control or alleviate heart failure symptoms by using evidence-based treatment and therefore maintain or improve quality of life.
The service is provided in community clinics across Liverpool. Home visits are also available for those with severe symptoms or who are housebound.
The Heart Failure Team will also provide education and supportive management to clinicians.
